This document exists to make it completely clear — the mechanics, the math, and the realistic path to income that consistently outperforms what you've been earning in healthcare.
A Different Kind of Income
You've spent your career being paid for your time. Shift comes, shift ends, paycheck arrives. What you made was fixed — regardless of how skilled you were, how efficient you were, or how much your patients loved you. That model protects the hospital. It doesn't reward you.
At Il Mulino, you're compensated on what you produce. The better you get — technically, relationally, commercially — the more you earn. There's no ceiling.
Why Commission — and Why It's Fair
Unlike a hospital or clinical setting, we don't have a waiting room full of patients who have to be here. Every client who walks through our door made a choice — to book, to invest in themselves, and to trust their provider. That dynamic creates real opportunity, but it also means we operate differently than traditional healthcare. There are real costs behind every treatment: the product in the syringe, the equipment in the room, the physician oversight, the facility, the marketing that brought that client in. We carry all of that. What that means for you is that you don't absorb the overhead of a slow week — we do. In exchange, when you have a great week, we don't cap what you take home. Commission isn't a workaround. It's the model that actually aligns our incentives — the better you perform, the better we both do.
How the Commission Structure Works
Your commission is calculated as a percentage of your total weekly sales. As your weekly revenue climbs through tiers, a multiplier is applied to your base commission rate — for every dollar earned in that tier range. Think of it as your rate of pay accelerating the more productive your week becomes.

If you're coming from a hospital or clinical setting, tipping isn't part of your world. It will be here. Aesthetics clients tip regularly — and when you deliver an exceptional experience, they tip generously. It's not guaranteed, and we don't build compensation models around it. But it's real, it's tax-reportable, and over the course of a week it adds up to something you'll notice.
Below are three realistic gratuity scenarios based on client volume and tip frequency. These sit entirely on top of your commission earnings.

You're finding your rhythm. We shift to every two weeks — enough space for you to operate independently, enough contact to course-correct before small habits become big ones. This is where we dig into client retention patterns, rebooking language, and identifying natural upgrade opportunities that serve your clients' goals.
By this point, you have a foundation. Monthly check-ins keep us aligned on your trajectory, flag anything worth addressing, and give you a standing channel to ownership whenever you need it — whether that's a clinical question, a difficult client situation, or a conversation about growing your schedule.
